CDHCI & continuing care at home

Typical flow

  1. Clinical assessment through AHS continuing care.

  2. Authorization and service package from your plan (e.g. Blue Cross where applicable).

  3. Choose a qualified provider on the Polymorphism marketplace.

  4. Visits, verified check-in/out, and billing all handled to match what your authorization requires.
